Areas of research:
The theme is on health financing systems and drug use in rural China. The aim was to study the association between health-care financing systems and health-care consumers’ drug use and health providers’ prescribing in six Chinese counties. A multistage sampling procedure was used in this cross-sectional study, health providers and patients were interviewed. We found that insured patients have significantly higher average medical (drug) costs per visit, and better access to expensive drugs and treatment such as hospitalization than uninsured. Health providers’ prescribing are influenced by patients’ insurance status and financial abilities, bonus payment mechanism, and market factors. The fee-for-service-based insurance leads to a higher growth of health care cost; to a shift from preventive to curative medicine and to a higher level of tertiary curative care cost. It also induces a higher per capita consumption of drugs.

The theme is on economic evaluation of medical device, aiming to develop tools and methodologies that effectively assess the value of a medical device product development at each stage of the product life-cycle, from pre-concept to mature product. At present, he is using a deterministic (and probabilistic) Markov model to make a preliminary assessment of the cost-effectiveness of total knee replacement (TKR) using computer-assisted systems (CAS) with that of TKR using conventional methods.

The theme was on control of tropical infectious diseases. Research on community-based health insurance (CBI) as a strategy to enhance the utilization of disease control measures. In the project, I was responsible for the research on the introduction and implementation of CBI in Burkina Faso. CBI has been widely viewed as one of the technologies in health care to improve the utilization of health services in developing countries because it disassociates use from payment. This was in light of the negative effects of user-fees and lack of feasibility for social insurance. In this process, preference for a benefit package was elicited. The benefit package was then developed and people’s willingness to pay elicited (see Dong et al, 2003a; Dong et al, 2003b; Mugisha et al, 2002; Dong et al, 2004a; Dong et al, 2004b). I was also responsible for time preference study in Burkina Faso.

Research:
Technology assessment for assisted reproductive techniques (funded by Ministry of Health, principal investigator). We studied the problems existed in the use of assisted reproductive techniques in China and provided suggestions to the government for how to reasonable use the techniques (see Dong et al, 1998g).

Hospital Internal Management Reform (funded by Ministry of Health, principal investigator). In order to meet the need of government for reforming hospitals, we studied the situation of hospital internal management and provided some measures to improve the efficiency and quality of hospitals (see Dong et al, 1998c; Dong et al, 1998f).

The Effects of Health Financing Systems on the Use and Distribution of Drugs in Rural China (Sida/SAREC project, principal investigator). To study the influence of user fees and insurance on the use of drugs, on the health-seeking behavior of patients and on the prescribing behavior of health providers (see Dong, 2000; Dong et al, 1999a; Dong et al, 1999b; Dong et al, 1999c; Dong et al, 1999d; Dong, 2003; Dong et al, 1998d; Dong et al, 1997c; Dong et al, 1997d).

Tropical Disease Control in the Transition: Impact of Charges in the Financing of Health Services on Schistosomiasis Control in China (WHO, TDR project, co-principal investigator). The purpose of this study was to know if the charges in the financing of health services on Schistosomiasis control could influence the cares people got for prevention and treatment (see Jiang & Dong, 1997).

Health Care Financing in Transitional Societies: Impact on Equity of User Fees in Chinese Health Care (Sida/SAREC project, principal investigator). From finance and use of health services points of view, we studied the impact on equity of user fees in six counties in rural China (see Dong, 2000; Bogg et al, 1996; Dong et al, 1997a; Dong et al, 1997b).

Development of Health Human Resource in Rural China (World Bank Health IV Project in China, researcher). We studied the problems of health human resources in quality and quantity existed in the levels of township and village in rural China. We also designed the strategies to resolve the problems by training, providing necessary equipment and technical supports from higher level health facilities (see Dong et al, 1997e; Dong et al, 1997f; Dong et al, 1995a; Dong et al, 1995c; Dong et al, 1994b; Dong et al, 1994c).

Demand and Supply of Health Human Resources (funded by Ministry of Health, researcher). We studied what was the situation of health human resources in rural China and what kind of health human resources would be needed in the future (see Dong et al, 1998a; Dong et al, 1998b; Dong et al, 1997g; Dong et al, 1997h).

Human Resources in Rural Primary Health Care (funded by Ministry of Health, researcher). We studied what was the situation of human resources in rural primary health care and what were the current human resources problems (see Dong et al, 1998e; Dong et al, 1994f).

Health Services Studies in Shanghai County (Minhang district) (funded by Ministry of Health, researcher). We studied in this research health needs, demands and supply of health services in Shanghai county, Shanghai (see Dong et al, 1994d; Dong et al, 1994e; Dong et al, 1989a; Dong et al, 1989b; Dong et al, 1988b; Dong et al, 1988d; Dong et al, 1987f).

Health Services Studies in Rural China (funded by Ministry of Health, researcher). We studied in this research health needs, demands and supply of health services in rural China (see Dong et al, 1995b).

Shanghai Development Strategy Research ----- Health Strategy Research (funded by Shanghai government, researcher). As a researcher I was responsible for the sub-study on the effects of ‘one child’ bonus policy (see Dong et al, 1987a).

Medical Care Finance in Rural China (funded by Ministry of Health, researcher). In this study, we studied the problems rural China faced in financing health care after the collapse of rural collective medical system in 1980s. I joined the design, data collection.

Human Resource and Finance of Maternal and Child Health Care (UNICEF project, researcher). As a researcher, I was responsible for the sub-study of financial flow of Maternal and Child Health Care (papers were not allowed to be published).

Cost-Effectiveness Analysis of Family Planning Programs in China (funded by Ministry of Health, master student). As a master student I designed this research, collected the data and did the data analysis. Based on the results I wrote my master dissertation (see Dong et al, 1988a; Dong et al, 1988c; Dong et al, 1987b; Dong et al, 1987c; Dong et al, 1987d; Dong et al, 1987e; Dong et al, 1987g; Dong et al, 1986a; Dong et al, 1986b; Dong et al, 1990a; Dong et al, 1990b; Dong et al, 1989c; Dong et al, 1987h).

The purpose was to know if the charges in the financing of health services on Schistosomiasis control could influence the care people got for prevention and treatment.

We wrote and won a proposal "The Effects of Health Financing Systems on the Use and Distribution of Drugs in Rural China " from SAREC. The purpose was to study the influence of user fees and insurance on the use of drugs, on the health-seeking behavior of patients and on the prescribing behavior of health providers.

We wrote and won a proposal " Health Care Financing in Transitional Societies: Impact on Equity of User Fees in Chinese Health Care " from SAREC. The objective was to analyze the equity from the financial side and health care utilization side.
